Attention parents and students:

Clint ISD has gone from a paper physical packet to an online/digital form process for high school athletic programs.  The physical form itself is the only paper form you will need to either print out or pick up from the school; unless you use an inhaler or any other medication on campus then a medicine permit signed by the parent and medical provider will also need to be printed from the below website or picked up from the high school. All other forms will be filled out online. The forms can be found and filled out at the following website:https://clintisd.rankonesport.com/ 

Please be sure to read the instructions on the website before you start filling out the forms. Both student and parent will need to sign the forms online and before turning in the completed physical form to the athletic trainer at the respective high school
