• Student’s records from school most recently attended- 

     

    • Withdrawal Form- If a student was previously enrolled, a withdrawal form should be requested.  

    This is used to avoid dual campus enrollment 

     

    • Report card/Transcript- Should include most recent grades and grade level in order to assist  

    with correct placement of the student 

     

     

     

    *If a student is missing any of the above mentioned documents (birth certificate, withdrawal forms  

    from previous campus, POR, etc.) please contact PEIMS Services to assist with obtaining documents.  

    Students should not be turned away unless nurse indicates they are not clear to register
