• Records Requests

    • Student records should be transferred via the Texas Records Exchange (TREx) if within the state of Texas 
    • Below is the minimum set of information that must be transferred when a student moves within Texas (by law, must be transferred within 10 days of request): 
    •   SSN or State-Approved alternate ID last reported through PEIMS 
    •   Current SSN or State ID, if different from above 
    •   Local student ID number, if applicable 
    •   Campus name and phone number 
    •   First, Middle, and Last Name with Generation Code 
    •   Sex, ethnicity and date of birth 
    •   Current grade level 
    •   Immunization records 
    •   Home Language Survey, HLS 
    •   ESL/LEP Documents (Parent consent, Denial, exit forms, etc.) 
    •   GT Parental approval for services 
    •   State assessment instrument results 
    •   Early Reader Indicator Code (elementary level) 

     

    Students Enrolling with Special Population Participation 

     

    •  Special Education information for new students- Identifies students who have received special  

    education services from prior school district. Completed forms must be forwarded to campus  

    Diagnostician for processing 

    •  Notify administrator overseeing Special Education, Diagnostician, student’s counselor and Mary  

    Gomez in SPED Dept. of students who enroll and indicate participation in SPED 

    •  Notify LPAC Clerk and administrator overseeing LEP when a new or returning 

    student indicates participation in LEP and/or has a qualifying response on HLS
