Records Requests
- Student records should be transferred via the Texas Records Exchange (TREx) if within the state of Texas
- Below is the minimum set of information that must be transferred when a student moves within Texas (by law, must be transferred within 10 days of request):
- SSN or State-Approved alternate ID last reported through PEIMS
- Current SSN or State ID, if different from above
- Local student ID number, if applicable
- Campus name and phone number
- First, Middle, and Last Name with Generation Code
- Sex, ethnicity and date of birth
- Current grade level
- Immunization records
- Home Language Survey, HLS
- ESL/LEP Documents (Parent consent, Denial, exit forms, etc.)
- GT Parental approval for services
- State assessment instrument results
- Early Reader Indicator Code (elementary level)
Students Enrolling with Special Population Participation
- Special Education information for new students- Identifies students who have received special
education services from prior school district. Completed forms must be forwarded to campus
Diagnostician for processing
- Notify administrator overseeing Special Education, Diagnostician, student’s counselor and Mary
Gomez in SPED Dept. of students who enroll and indicate participation in SPED
- Notify LPAC Clerk and administrator overseeing LEP when a new or returning
student indicates participation in LEP and/or has a qualifying response on HLS
