Both forms below are required to provide access. Please check forms for completeness. Incomplete forms or forms with missing signatures will not be processed.
When completed and signed, both forms must be sent to firstname.lastname@example.org for processing.
A completed and approved District System Security Access (DSSA) Form is needed by any user conducting official business for or with the district that are not district employees and who may need access to District Information Resources.
*Note Volunteers must have a completed and approved volunteer application on file with the Dept. of Human Resources.
All individuals granted access to district technology and information resources must be aware of and agree to abide by the acceptable use policy.
District staff and students acknowledge the AUP annually within the student and employee handbooks.
