Transcript Course Request Form

This form is to be submitted to PEIMS Services when requesting to add a transfer course that does not already exist in the course master for the year the student earned the transfer credit

Fill out the indicated fields, including the length and credits earned. You will enter the school year the student earned the credit as well as the service ID of the transfer course. If the local ID does not populate, you will transfer the information to the bottom portion.
