Documents Needed for New Students

birth certificate;

immunization record (immunization information);

child's social security card; (optional)

current utility bill; (gas, water or electric bill)

driver's license or I.D. of parent;

proof of income for Pre-K;

and any previous school records or report cards.

You may now attach documents or photos using your computer or smartphone to complete the online registration process.
Documents to provide or attach if necessary:
1. Proof of Address (gas, water, or electric bill only required for change of address)
2. Current shot/immunization records.
3.Power of Attorney if expired.
You must complete all steps of the process and submit your child's registration. Upon completion you will receive a confirmation email
that your application was submitted.
