Documents to provide or attach if necessary:
1. Proof of Address (gas, water, or electric bill only required for change of address)
2. Current shot/immunization records.
3.Power of Attorney if expired.
You must complete all steps of the process and submit your child's registration. Upon completion you will receive a confirmation email
that your application was submitted.
